Thank you for taking the time to register your child for Lacamas Conference Center's 2008 kids camp! Please fill out the form below in its entirety. After completing and submitting the registration form, you will be prompted to pay your child's 2008 camp registration fee (outlined above). As parent or legal guardian, I give the above named student my permission to attend a Lacamas Campground Camp. In the event of illness or injury, I authorize the camp staff to provide emergency care by the camp nurse, local medical staff or hospital, in case I cannot be reached. I understand that the camper insurance is secondary to my own primary coverage, for which I am responsible. I hereby verify that all immunizations are current and the above information is complete and accurate. I agree to notify Lacamas Campground of any changes prior to the camp session. I understand that it is my responsibility to notify the camp regarding food allergies my child has prior to camp. Registration fees are non-refundable. Acceptance and participation in Lacamas Camping programs are the same for everyone regardless of gender, race, color, or national origin. Media clips of participant may be used for Lacamas Campground promotional purposes.
